Cancellations

All cancellations must be in writing. You can send your cancellation using one of the following methods. 

• Email us your cancellation at billing@flmtgif.org. Include your full name, and account login or email address with Family Life.
• Fax us your cancellation at 877-419-8299.
• Mail us your cancellation via USPS. Send your snail mail to

Please include your full name, and account login or email address with Family Life. If possible, please include a reason why you are canceling so we can evaluate and improve our service.
